Resumen
En el ámbito de la salud peruana, los hospitales del Seguro Social de Salud,
localizados estratégicamente a lo largo del Perú, se encargan de satisfacer la gran demanda de
salud existente brindando prestaciones de salud en prevención, promoción y recuperación
entre la población asegurada. En ese sentido, el presente trabajo de investigación ha
elaborado un Business Consulting para el Hospital II-1 Luis Heysen Incháustegui en la
ciudad de Chiclayo. El propósito de esta consultoría es conocer los factores externos e
internos que inciden en el desempeño de la gestión del nosocomio relacionado a un alto
embalse quirúrgico, que compromete al establecimiento en no conseguir sus objetivos de
largo plazo. Las reuniones con parte de la Dirección del Hospital y el personal asistencial
coinciden a que la causa raíz del problema está vinculada con procesos y procedimientos del
flujograma de atención del paciente quirúrgico programado.
En base a la literatura investigada, se propone implementar un programa innovador
denominado “Cirugía al Dia”, iniciativa que establece poner en marcha intervenciones
quirúrgicas electivas que no requieren ocupar una cama hospitalaria postoperatoria. De esta
manera, la ejecución del plan de implementación espera impactar de manera favorable en la
gestión de las cirugías en términos de reducción de costos operativos por paciente,
satisfacción en calidad de servicio al paciente y disponibilidad de mayor número de
programaciones de intervenciones. Por último, la elaboración de una serie de estimados para
tres años, corroboran lo esperado, logrando así alcanzar una mayor producción que resulta en
8127 intervenciones quirúrgicas adicionales. Además, se obtiene una reducción del embalse
quirúrgico del 30%; en otras palabras, el número de asegurados en lista de espera para un
servicio de cirugía baja a 665. Y se alcanza un ahorro por concepto de pagos de pacientes
fuera de Lambayeque en un rango de S/ 1.2 millones y S/ 1.6 millones anuales.
In the field of Peruvian health, the Social Health Security hospitals, strategically located throughout Peru, are in charge of satisfying the great demand for existing health by providing health benefits in prevention, promotion and recovery among the insured. In this sense, the present research work has prepared a Business Consulting for Hospital II-1 Luis Heysen Incháustegui in the city of Chiclayo. The purpose of this consultancy is to know the external and internal factors that affect the performance of hospital management related to a high surgical reservoir, which commits the establishment not to achieve its long-term objectives. The meetings with part of the Hospital Directorate and the healthcare personnel agree that the cause of the problem is linked to processes and procedures of the care flow chart of the scheduled surgical patient. Based on the researched literature, it is proposed to implement an innovative program called "Surgery to the Day", an initiative that establishes the implementation of elective surgical interventions that do not require occupying a postoperative hospital bed. In this way, the execution of the implementation plan expects to have a favorable impact on the management of surgeries in terms of reduction of operating costs per patient, satisfaction in the quality of patient service and availability of a greater number of intervention programs. Finally, the preparation of a series of estimates for three years corroborates what was expected, thus achieving a higher production resulting in 8127 additional surgical interventions. In addition, a reduction of the surgical reservoir of 30% is obtained; In other words, the number of insured persons on the waiting list for a surgery service drops to 665. And there is savings for patient payments outside Lambayeque in a range of S / 1.2 million and S / 1.6 million annually.
