Resumen
El presente proyecto de innovación buscar mejorar el acceso a la atención integral de salud
de las personas adultas mayores (PAM) en las Instituciones Prestadoras de Servicios de
Salud (IPRESS) de primer nivel de Lima Norte mediante la Valoración Clínica del Adulto
Mayor (VACAM) que permite diagnosticar el estado de salud de los adultos mayores. Por
ello, para lograr el acceso de los adultos mayores a una atención médica apropiada acorde
a sus necesidades, se identificó que las causas relacionadas al macroentorno en el que
viven, la estructura del sistema de salud, la ubicación de los servicios de salud, las
características de la infraestructura de las IPRESS y los recursos humanos que brindan los
servicios de atención.
Identificada la problemática y sus causas, estas se jerarquizaron y evaluaron, planteándose
el siguiente desafío de innovación “¿Cómo podemos mejorar el modelo de atención
médica a las PAM en las IPRESS de primer nivel de Lima Norte para brindar servicios de
calidad que se encuentren adecuados a la norma técnica vigente?”, se revisó experiencias
previas para su atención y como resultado se planteó el “Programa ruta móvil y atención
domiciliaria para la atención integral de salud de las PAM afiliadas al SIS de la DIRIS de
Lima Norte”, el cual cuenta con los siguientes componentes; el planeamiento de la
intervención, la confirmación telefónica para la atención de la PAM, la atención integral
a la PAM por la ruta móvil itinerante y domiciliaria, y el seguimiento del plan de atención
y tratamiento de la PAM.
Thisinnovation Project aimsto improve access of elderly people (PAM) to comprehensive healthcare service in the first-level Health Care Providers (IPRESS) in North Lima by means of the Clinical Assessment of Elderly People (VACAM), which allows diagnosing their health state. Therefore, in order to ensure that elderly people have access to appropriate medical care that meets their needs, we identified the causes related to the macro-environment in which they live, the structure of the health system, the location of health services, the characteristics of the IPRESS infrastructure and the human resources that provide health care services. After identifying the issues and their causes, these were classified and evaluated, giving rise to the following innovation challenge: “How can we improve the medical care model provided to PAM in the first level IPRESS in North Lima so as to offer quality services that meet the current technical standards?”. Previous experiences were examined and as a result the “Mobile Route and Home Care Program for the Comprehensive Health Care for PAMs affiliated with the SIS (Comprehensive Health Insurance) from the North Lima DIRIS (Integrated Health Networks Directorate)” was designed, which has the following components: intervention planning, telephone confirmation for the care of PAMs, comprehensive care for PAMs by means of the itinerant and home mobile route, and follow-up of the care and treatment plan for PAMs.
