Resumen
La presente investigación es un estudio sistemático de caso único de un paciente con
sintomatología ansiosa en un proceso de psicoterapia psicoanalítica breve. Se tuvo como
objetivo identificar y describir el patrón relacional y los conflictos internos a la base de
dicha sintomatología, así como aproximarse a la manera en que estas variables se
entrelazan. La intervención basada en el modelo psicoterapia dinámica interpersonal
breve (DIT), se centró en el trabajo del foco afectivo interpersonal, que permitió a su vez
dar cuenta de las variables de estudio. Para la delimitación del material clínico constituido
por los episodios relacionales de las tres primeras sesiones, se aplicó el método CCRTLU-S; y a partir de ellos, se procedió a conceptualizar el Eje II Relación y Eje III Conflicto
del OPD-2. Los resultados evidencian, un patrón de relación disfuncional en el
consultante, que incluye la necesidad de aferrarse, y al mismo tiempo, de tomar distancia,
al percibir que el objeto es demandante en sus necesidades o no se encuentra disponible
para él. Los conflictos giran en torno a una dependencia significativa hacia el objeto; y a
su vez, una independencia forzada para protegerse de la cercanía. Los conflictos más
significativos fueron Individuación versus Dependencia; y Protección y cuidado versus
Autarquía.
This investigation is a systematic study of a single case of a patient with anxiety in a process of brief psychoanalytic psychotherapy. It aims to identify and describe the relational pattern and the internal conflicts that are at the basis of this symptomatology, as well as to approach the way these two variables engage each other in the emergence of anxiety. The intervention based on the model of brief interpersonal dynamic psychotherapy (DIT), focused on the work of the interpersonal affective focus, from which was possible to identify the study variables. For the analysis of the clinical material constituted by the relational episodes of the three first sessions, the CCRT-LU-S was applied; and from them, it was possible to conceptualize the Axis II Relation and Axis III Conflict from OPD-2. The results show a maladaptative relational pattern that includes a need to hold on, and at the same time, to take distance, by perceiving that the object is demanding in its needs, or not available for him. In that line, the conflicts were related with a significant need of dependence; and at the same time, with a forced independence in order to protect himself from closeness. The most significant conflicts were Individuation versus Dependence and Protection and Care wishes versus Autarchy.
