La trayectoria de las intervenciones verbales de un terapeuta y su contribución al cambio psíquico en un proceso de psicoterapia psicoanalítica focal

Resumen
Esta es una investigación de caso único en psicoterapia y tiene por objeto de estudio las
intervenciones verbales de un terapeuta en un contexto conversacional de mutua influencia con una
paciente y pretende describir y analizar su contribución al cambio subjetivo, tomando por categorías
de análisis sus estrategias de intervención, sus elecciones léxicas y posiciones yo. Para este fin se
identificó el foco temático correspondiente al Indicador de Cambio de mayor jerarquía identificado
en todo el proceso según el modelo de cambio de Krause et al. (2006) y se rastreó
retrospectivamente en los Episodios de Cambio previos las intervenciones del terapeuta que
contribuyeron a construirlo, atendiendo a su trayectoria en función al diálogo con la paciente. Se
elaboró un análisis basado en la organización secuencial del habla y un análisis temático de los
resultados. Los resultados mostraron que la participación subjetiva del terapeuta puede contribuir al
cambio subjetivo de la paciente. Así, algunas conclusiones centrales son que su alternancia entre
cuestionar y reconocer la perspectiva del paciente, formular sus propuestas de manera no
conclusiva, para confirmarse o modificarse en el diálogo con éste y el uso de metáforas, contribuyen
al cambio. Así, se presentan evidencias de que el terapeuta puede participar subjetivamente en el
proceso psicoterapéutico y contribuir al cambio subjetivo del paciente desde una concepción bipersonal
de la psicoterapia, atento a los procesos singulares en las experiencias del paciente.
This research is a single case study in psychotherapy and it’s object is the verbal interventions of one psychotherapist in a context of mutual influence with a patient. It pretends to describe and analyze its contribution to subjective change. It’s categories of analysis are the therapist’s strategies of intervention, his lexical choices and self-positions. To do so, the topic subject of speak related to the Generic Change Indicator of highest hierarchy in the whole process according to the model of change by Krause et al., (2006) was identified and then tracked down in the therapist’s interventions which contribute to build it among the previous Change Episodes, attending to its trajectory in relation to the dialogue with the patient. The analysis was based on the sequential organization of talk and, later, a thematic analysis of the data results. Results showed that subjective participation of the therapist can contribute to the patient’s therapeutic change. Thus, some main conclusions are that the process of the therapist’s regulation, altering between acknowledgments and questioning of the patient’s perspective, formulating his own interventions in non-conclusive ways, which gain confirmation or the need of being modified from the dialogue with the patient, and the use of metaphors, contributes to psychic change. Thus, this shows evidence that supports the possibility of the subjective participation of the therapist in the psychotherapeutic process that contributes to the patient’s subjective change from a conception of psychotherapy as a bi-personal process.
