Cambio en los patrones relacionales en un caso de psicoterapia breve de una consultante con depresión

Resumen
La depresión es la primera causa de discapacidad laboral, personal y familiar en el planeta
(Organización Mundial de la Salud [OMS], 2020) y genera altos costos a nivel de salud pública
(Ministerio de Salud [MINSA], 2018; Organización Panamericana de la Salud [OPS], 2014). Por
lo tanto, en el contexto peruano, es importante buscar estrategias útiles, de menor costo como las
terapias focales y breves, que ayudan al abordaje con objetivos específicos para lograr aliviar la
depresión y sufrimiento significativo en el consultante; así como, métodos de investigación
clínica que lo sustenten. En este marco, el presente estudio busca explorar el cambio en los
patrones relacionales de una consultante con depresión, en un proceso de psicoterapia
psicoanalítica breve, basado en la terapia dinámica interpersonal (TDI). Con este propósito, se
realiza un estudio de caso único sistemático, mediante una metodología cuantitativa. Para el
análisis del material clínico, se utiliza el método del Tema Central de Conflicto Relacional
(CCRT-LU-S) para identificar los patrones relacionales centrales (PRs) de la fase inicial y final
del tratamiento, examinando los cambios a partir de su contraste. En los resultados, se observan
cambios en los diferentes componentes de los PRs, tanto en los deseos (Ds), como en la
respuesta del otro (RO) y las respuestas de sujeto (RSs); siendo el de mayor cambio significativo,
la respuesta del sujeto hacia el self (RSS). Se concluye que las representaciones de sí misma y
del otro (DOS, ROS y RSs), se actualizan en el tratamiento de una psicoterapia psicoanalítica
breve, basada en el modelo de la TDI.
Depression is the leading cause of work, personal and family disability on the planet (World Health Organization [WHO], 2020); it generates high costs at public health level (Ministry of Health of Peru [MINSA], 2018); Pan American Health Organization [PAHO], 2014). Therefore, it is important in the Peruvian context to seek useful, lower-cost strategies, such as short and focal therapies, which help the approach with specific objectives, in order to alleviate depression and significant suffering in the consultant. Clinical research methods to support it are important as well. In this framework, the present study seeks to explore the change in the relational patterns of a consultant with depression, in a brief psychoanalytic psychotherapy process based on dynamic interpersonal therapy (DIT). For this purpose, a study of a systematic single case is carried out, through a quantitative methodology. For the analysis of the clinical material, the Core Conflictual Relationship Theme (CCRT-LU-S) method is used to identify the central relational patterns (PRs) of the initial and final phases of the treatment, examining the changes based on their contrast. In the results, changes are observed in the different PRs components: in wishes (Ws [Ds]), in the response from the others (RO) as well as in the responses from the subject (RSs). The greatest significant change is found in the response of the subject towards the self (RSS). It is concluded that the representations of herself and the other (WOS [DOS], ROS and RSs), are updated in the treatment of a brief psychoanalytic psychotherapy, based on the DIT model.
