Una revisión de las APP en salud en América Latina
Abstract
La Agenda 2030 para el Desarrollo Sostenible incluye un compromiso con la cobertura sanitaria universal (CSU), lo que lleva a un intenso debate sobre cómo se financiarán y prestarán los servicios de atención de la salud. El paradigma político más dominante promueve el uso de las finanzas privadas para cumplir con los Objetivos de Desarrollo Sostenible (ODS). Las asociaciones público-privadas (APP) son fundamentales para esta estrategia y hay un número creciente de APP en salud en toda América Latina. Con un enfoque específico en el caso de Perú, el documento de trabajo examina los debates más amplios en torno a las APP en salud, prestando especial atención al auge de las APP hospitalarias. En la actualidad, Perú cuenta con dos hospitales APP, proyectos que implicaron la construcción y operación clínica de nuevos hospitales, cada uno con los correspondientes centros de atención primaria y de urgencias, al tiempo que se planean varios más para el futuro. Sin embargo, a pesar de su amplia promoción, todavía hay muy poca evidencia que respalde las afirmaciones de que las APP pueden abordar eficazmente las desigualdades sanitarias o que ofrecen “valor por dinero”. Dada la naturaleza ya fragmentada del sistema de salud de Perú y las históricas desigualdades de género, de raza y geográficas, siguen habiendo serias dudas sobre la viabilidad de utilizar las APP para el logro eficaz de la CSU y de los ODS conexos.
